Prepare for Your Visit

Please arrive five minutes prior to the appointment time. The appointment may be rescheduled if you arrive more than 15 minutes after the scheduled time.

What to Bring

Please bring the following to your visit:

• Insurance card
• Photo ID
• Medical records related to visit
• Copayment
  Please be aware that you will be charged for one copayment per physician, which will be two copayments.

Lab Work:

The treatment follow-up plan may include lab work. Staff will give you options and instructions for laboratory testing.
