Appointments and Scheduling
We strive to make the appointment and scheduling process easy and convenient for our patients and their families. To ensure we deliver the best experience possible, please have the following items ready when you call to schedule your appointment:
- General patient information and pertinent medical history.
- Prescription from your child’s physician.
- Your child’s insurance information.
- Reason for your child’s visit.
Our patient access representative may also ask you to fax us the patient’s prescription information so we may request authorization from the patient’s insurance company and appropriately verify the patient’s insurance information.
