Welcome to Orlando Health Arnold Palmer Hospital for Children Center for Orthopedics!

Our goal is to provide the highest quality of care possible in a reasonable time frame. We understand and respect the importance of your time. We strive to make your visit as seamless and efficient as possible. Please read the following guidelines to assist us in this goal:

Please arrive 15 minutes prior to your appointment time in order to process your paperwork, pay your copay/deductible and or/coinsurance. If you are more than 20 minutes late to your appointment we will possibly need to reschedule you.

Please fill out the attached paperwork completely to avoid delaying your appointment time. Bring your insurance card and photo ID. Please make sure that you contacted your referring physician to send us your records concerning the diagnosis in which your are referred for, including any radiology films or diagnostic studies that have been done. These records can be sent to us via fax at (321) 841-3049. Hard copy radiology films or disks can be brought to the visit for the physician to view.

If your insurance requires a referral this should be obtained by you from your referring physician or primary care physician and faxed to us at (321) 841-3049 prior to your scheduled appointment date. If the referral is not received prior to the time of your appointment, you may be asked to reschedule.
